    Undergraduate medical students' behavioural intentions towards medical errors and how to handle them: A qualitative vignette study

    Objectives In undergraduate medical education, the topics of errors in medicine and patient safety are under-represented. The aim of this study was to explore undergraduate medical students' behavioural intentions when confronted with an error. Design A qualitative case vignette survey was conducted including one of six randomly distributed case scenarios in which a hypothetical but realistic medical error occurred. The six scenarios differed regarding (1) who caused the error, (2) the presence of witnesses and (3) the consequences of the error for the patient. Participants were asked: â € What would you do?". Answers were collected as written free texts and analysed according to qualitative content analysis. Setting Students from German medical schools participated anonymously through an online questionnaire tool. Participants Altogether, n=159 students answered a case scenario. Participants were on average 24.6 years old (SD=7.9) and 69% were female. They were undergraduate medical students in their first or second year (n=27), third, fourth or fifth year (n=107) or final year (n=21). Results During the inductive coding process, 19 categories emerged from the original data and were clustered into four themes: (1) considering communication; (2) considering reporting; (3) considering consequences; and (4) emotional responsiveness. When the student him/herself caused the error in the scenario, participants did mention communication with colleagues and taking preventive action less frequently than if someone else had caused the error. When a witness was present, participants more frequently mentioned disclosure of the error and taking actions than in the absence of a witness. When the outcome was significant to the patient, participants more often showed an emotional response than if there were no consequences. Conclusions The study highlights the importance of coping strategies for healthcare professionals to adequately deal with errors. Educators need to introduce knowledge and skills on how to deal with errors and emotional preparedness for errors into undergraduate medical education

    eta - eta' - glueball mixing

    We have revisited glueball mixing with the pseudoscalar mesons in the MIT bag model scheme. The calculation has been performed in the spherical cavity approximation to the bag using two different fermion propagators, the cavity and the free propagators. We obtain a very small probability of mixing for the eta at the level of $0.04-0.1% and a bigger for the eta' at the level of 4-12%. Our results differ from previous calculations in the same scheme but seem to agree with the experimental analysis. We discuss the origin of our difference which stems from the treatment of our time integrations.Comment: 21 pages, 7 figure

    Decoherence of Einstein-Podolsky-Rosen steering

    We consider two systems A and B that share Einstein-Podolsky-Rosen (EPR) steering correlations and study how these correlations will decay, when each of the systems are independently coupled to a reservoir. EPR steering is a directional form of entanglement, and the measure of steering can change depending on whether the system A is steered by B, or vice versa. First, we examine the decay of the steering correlations of the two-mode squeezed state. We find that if the system B is coupled to a reservoir, then the decoherence of the steering of A by B is particularly marked, to the extent that there is a sudden death of steering after a finite time. We find a different directional effect, if the reservoirs are thermally excited. Second, we study the decoherence of the steering of a Schr\"odinger cat state, modeled as the entangled state of a spin and harmonic oscillator, when the macroscopic system (the cat) is coupled to a reservoir

    The Stochastic Toolbox User's Guide -- xSPDE3: extensible software for stochastic ordinary and partial differential equations

    The xSPDE toolbox treats stochastic partial and ordinary differential equations, with applications in biology, chemistry, engineering, medicine, physics and quantum technologies. It computes statistical averages, including time-step and/or sampling error estimation. xSPDE can provide higher order convergence, Fourier spectra and probability densities. The toolbox has graphical output and χ2\chi^{2} statistics, as well as weighted, projected, or forward-backward equations. It can generate input-output quantum spectra. All equations may have independent periodic, Dirichlet, and Neumann or Robin boundary conditions in any dimension, for any vector field component, and at either end of any interval.Comment: User manual for xSPDE software on Github, at https://github.com/peterddrummond/xspde_matlab.     Distinct polyadenylation landscapes of diverse human tissues revealed by a modified PA-seq strategy

    Background: Polyadenylation is a key regulatory step in eukaryotic gene expression and one of the major contributors of transcriptome diversity. Aberrant polyadenylation often associates with expression defects and leads to human diseases. Results: To better understand global polyadenylation regulation, we have developed a polyadenylation sequencing (PA-seq) approach. By profiling polyadenylation events in 13 human tissues, we found that alternative cleavage and polyadenylation (APA) is prevalent in both protein-coding and noncoding genes. In addition, APA usage, similar to gene expression profiling, exhibits tissue-specific signatures and is sufficient for determining tissue origin. A 3? untranslated region shortening index (USI) was further developed for genes with tandem APA sites. Strikingly, the results showed that different tissues exhibit distinct patterns of shortening and/or lengthening of 3? untranslated regions, suggesting the intimate involvement of APA in establishing tissue or cell identity. Conclusions: This study provides a comprehensive resource to uncover regulated polyadenylation events in human tissues and to characterize the underlying regulatory mechanism

    AbstractWe report late diagnoses of cystic fibrosis (CF) in two men aged 61 and 65 years. At the time of presentation, both patients had significant pulmonary disease. In each case two CFTR gene mutations were identified, including R117H on a background of a poly T genotype of 7T/9T. Patients with two identified CFTR mutations which include the R117H/7T anomaly should be followed up routinely as they remain susceptible to severe lung disease

    Rationality as the Rule of Reason

    The demands of rationality are linked both to our subjective normative perspective (given that rationality is a person-level concept) and to objective reasons or favoring relations (given that rationality is non-contingently authoritative for us). In this paper, I propose a new way of reconciling the tension between these two aspects: roughly, what rationality requires of us is having the attitudes that correspond to our take on reasons in the light of our evidence, but only if it is competent. I show how this view can account for structural rationality on the assumption that intentions and beliefs as such involve competent perceptions of downstream reasons, and explore various implications of the account
